STEP ONE: ASSESS

We work with families who are trying to figure out how best to care for their loved one who has Alzheimer’s Disease or a Dementia diagnosis. We do not diagnose anyone, we assist their families to assess the current and future needs while aging in place.

STEP TWO: IDENTIFY

After our interview with our client and family; we will determine if aging in place with the family or transitioning to an outside care facility is best. Once that collective decision is made; we will identify the most urgent needs first. We will identify:

  • Fall risk concerns

  • Determine if  additional care assistance is needed in the home

  • Identify the best 1-4 care facilities based on our 8 point criteria for possible transition out of the home. 

  • Have family complete “Knew You” Form

STEP THREE: DESIGN

We use all of the pertinent information gathered from our assessments and create a strategic plan based on our client and family’s needs. We finalize the plan with recommendations listed from most urgent to least urgent for you and your family to review.

STEP FOUR: EDUCATE

Education is the key to understanding your loved one, so we initiate education and provide tips upon meeting. We also offer a list of community resources for family support groups and other needs to assist caregivers and families with additional concerns.
