When you are transitioning from a care facility, we can assist in making sure that it is a smooth process for and your loved ones. 

POST HOSPITAL CONSULTATION

Assess. Identify. Design. Educate

STEP ONE: ASSESS

We recommend initiating this consultation 1-2 weeks prior to the estimated discharge date is considered elective or outpatient. We will schedule a meeting to discuss your needs and the expected care facility’s length of stay. Our interdisciplinary team approach is used to assess a complete list of possible concerns in order of most urgent.

STEP TWO: IDENTIFY

Since discharge needs for everyone vary; we identify the safest plan with services to ease your transition back home. In collaboration with the discharge planners we: 

  • Assess any barriers to safely enter and exit the home. 

  • Discuss possible DME needs and delivery dates and times. 

  • Arrange transportation home and to aftercare appointments for the first week.  

  • Family education for best tips on how to assist clients safely.

STEP THREE: DESIGN

We design a strategic plan based on your specific functional needs as well as services to increase the ease of your transition back home. Our plans are designed with your collaboration for the best outcomes. Our plans include: 

  • Monitor all the recommendations that are completed prior to your arrival home. 

  • Review all DME orders and delivery dates to ensure all orders are consistent with recommendations.

  • Confirm the dates and times of all aftercare appointments for the first 2 weeks.  

  • Discuss transportation needs to the first discharge medical follow-up appointment. 

  • Discuss any wrap-around services like house cleaning/laundry, grocery shopping, meal prep, medication reminders, and Wellness Checks that might be needed. 

  • Home Safety Evaluation and recommendations for home modifications.

STEP FOUR: EDUCATE

We review all discharge recommendations for your understanding; in our experience, we know clients are more invested in their care when they have a full understanding of the process. We want to ensure all conversations pertaining to our services are collaborative for the best outcomes.
